Medication Therapy Management
Medication therapy management (MTM) is a service provided by pharmacists and other health care providers to ensure the best therapeutic outcomes for patients. MTM includes five core elements: medication therapy review, personal medication record, medication-related action plan, intervention or referral, and documentation and follow up. The goal of MTM is to improve patient adherence with their prescribed medications which can lead to improved quality of life as well as better outcomes in chronic diseases such as diabetes mellitus or hypertension.
Our pharmacists work at the top of their license to take the time to provide quality clinical services. Mesilla Valley Pharmacy’s medication therapy management program includes these five core elements:
Medication therapy review – A complete medication assessment and evaluation of the patient’s medical history, including any past problems with medications. This includes a drug-drug interaction check to make sure that patients are not taking anything else which could increase or decrease the effects of their current prescribed drugs.
Intervention or referral – If the pharmacist determines that a medication-related issue is preventing the patient from maintaining optimal health, an alternative therapy may be suggested. For instance, if a person has been found to have diabetes mellitus Type II and their blood sugar levels are not being adequately controlled with oral medications, insulin injections could be recommended.
Personal medication record – The pharmacist creates a personalized medication record for the patient detailing all medications prescribed by every physician, any relevant allergies and adverse drug reactions. This records helps to identify potential problems that may arise with taking multiple drugs at once or on an ongoing basis.
Medication-related Action Plan – The pharmacist creates a medication-related action plan for the patient based on their personal medication record. This includes education and counseling about medications as well as guidelines to help avoid adverse drug reactions or issues with misuse of drugs such as overuse, mixing prescription pills with other substances like alcohol that could lead to unconsciousness. We aim to provide personalized solutions for our patients.
Documentation and Follow Up – The completed MTM assessment is documented in the patient’s medical file and the pharmacist will plan to implement and follow up on corrective actions that were discussed.
The evidence behind medication therapy management (MTM) has been shown to be effective for lowering systolic and diastolic blood pressure; lowering LDL cholesterol and other health indicators (e.g., glycosylated A1C, HbA1c); increasing patient knowledge; improving patient quality of life and medication adherence; and improving the safe and effective use of medications, including reducing therapeutic duplication.
